Here you will find a downloadable referral form that you can take to your doctor. We will need the doctor to fill in the top portion and sign. This is an insurance requirement if a patient has physical therapy as a covered benefit in their insurance plan or policy.

We honor any prescription for PT that is signed by a licensed medical doctor (MD, DO, or DDS) and including nurse practitioners and physician assistants (PA or NP). We cannot accept chiropractic referrals at this time in Michigan.
